Would've liked to respond to the survey, but I think it's dead. No way to send it after it's filled out.

It may indeed be closed, but I hit the page just now and if you scroll to the very end of the page there is a Submit button. Did you try that, or is that the problem -- does it error out on you? If so, you could email your responses to the email address provided, assuming that's still valid as well. We had no trouble responding to it initially.
